​​Everyone starts somewhere. This resource library gives teenagers and young adults with CP information to stay active, build confidence, and understand their health needs. Families will find tools that make navigating healthcare and adult life a little easier. Providers can explore outside perspectives to better support patients during the transition process. These resources are meant to spark learning, encourage independence, and help you share helpful ideas with others in the CP community.

Got Transition

A national resource center focused on helping teenagers and young adults prepare for adult healthcare. Their tools and checklists help patients, families, and providers plan realistically and communicate better during transition.  

 

Cerebral Palsy Research Network – Adult CP Toolkit

A detailed toolkit created with input from adults with CP and clinicians. It covers health, independent living, goal-setting, advocacy, and planning for adulthood. Great for long-term use.

 

United Cerebral Palsy – Adaptive Equipment for Daily Living

A clear overview of adaptive equipment options for mobility, communication, household tasks, and school/work. Helps families and young adults explore what supports independence in daily life.

 

NCHPAD – National Center on Health, Physical Activity and Disability (Exercise Videos & Programs)

Tons of videos, downloadable exercise guides, and adaptations for different physical abilities. They walk through routines visually and explain modifications.

 

Moving to Adult Healthcare – Transition Planning Resources

Created specifically for youth with developmental disabilities. Offers planning tools and tips for navigating adult medical systems and organizing your health information.

 

Assistive Technology for Daily Living

A practical guide to day-to-day adaptive tools such as bathroom supports, dressing aids, communication devices, and home modifications that support independence at all ages.

Bay Area Outreach & Recreation Program (BORP) – Virtual Fitness Classes

Live and on-demand adaptive fitness classes for people with physical disabilities. Options include seated workouts, chair yoga, dance, and strength training, all designed to be accessible for a range of ability levels.

 

DO-IT Center (University of Washington) – College & Transition Resources

A comprehensive information hub for students with disabilities navigating college. Covers self-advocacy, accommodations, assistive technology, accessibility, and transitioning to higher education.

The Lep Foundation- John Lepping Memorial Scholarship
